This excellent research paper demonstrates poorer outcomes, following keyhole surgery of the hip if there is also underlying cartilage damage. The MRI scans can give us some indication, but the only way off truly determining the extent off synovial cartilage damage is during keyhole surgery(arthroscopy) itself. This is graded using the Outerbridge scoring system (from 0 to 4), with grade 4 changes representing areas of exposed bone. This paper highlights the importance of addressing cartilage damage, as well as hip impingement and torn labrum when performing hip arthroscopy. The rehabilitation is different and may take longer if cartilage regeneration surgery is also performed.
